What benefits am I eligible for?

Full-time Groendyke employees are eligible for the following benefits;
Group Medical, including a major medical plan with Blue Cross and Blue Shield.
Two plans are available based upon eligibilty.

Included at no additional charge in both plans;
Vision, Prescription Plan and Life Policy.

In addition, employees can elect the following supplementary benefits;
401(k) with company match
Wellness Program
Group Dental
Short-term Disability
Long-term Disability
Flexible Spending Accounts - Medical and Dependent Care
Supplemental Term and Dependent Life Insurance
Critical Care, Accident and Whole Life Plans are available

I'm a new employee. When am I eligible to start my benefits?

You may enroll in benefit plans within 30 days of your date of hire. Your group insurance will be effective the first day of the month that you are actively at work following 90 continuous days of full-time employment.

Employees are eligible to participate in the 401(k) savings plan on the first day of the month following their employment date.

Critical Care, Accident and Whole Life Policies have a semi-annual enrollment.

I'm getting married. How will my benefits change?

If the Benefits Department receives your enrollment form(s) within 31 days of your marriage, you may be able to make the changes listed below.
Health and Dental Plans;
If the employee is already enrolled, you may;
Add or drop eligible dependents;
Cancel your coverage (if you are gaining coverage through your spouse).

If you are enrolling for the first time, please contact the Benefits Department for information and plan guidelines.

Flexible Spending Account - Medical or Dependent Care:
Enroll in an account(s);
Change your annual contributions (if you are gaining coverage through your spouse).

Supplemental Life:
If the employee is already enrolled, you may add spouse and/or dependent life policies upon approval from the carrier.

Beneficiary Information:
You may wish to review and update your beneficiaries under the Groendyke life Insurance Plans and the 401(k) to ensure they are current. Employees may do this at any time throughout the year.

What happens to my benefits when I have or adopt a child?

If the Benefits Department receives your enrollment form(s) within 31 days of your child's birth or adoption you generally may be able to make the changes listed below.

Health and Dental Plans;
As a result of adopting or having a child, you may;
Enroll in one of Groendyke's offered Medical Plan or Dental with employee and child or employee and family coverage.
If you are already enrolled, you may;
Add or drop eligible dependents;
Cancel your coverage (if you are gaining coverage through your spouse).

What is a flexible spending account?

The Plan provides each eligible employee with the opportunity to set aside part of his or her pay on a pre-tax basis to;

Provide for payment of qualified medical, dental or dependent care expenses on a tax-free basis. The Plan helps you because the benefit you elect is nontaxable. In addition, you save Social Security and Income Taxes on the amount of your salary reduction used to pay these expenses.
